ADA Certification Process

Requirements

Americans with Disabilities Act (ADA) regulations require an administrative process for individuals to request to be evaluated for ADA eligibility.

Process

Upon receiving a request for an ADA application, Battle Creek Transit will provide an ADA Application (Fillable PDF) and an Authorization of Physician or Health Care Provider Release of Information Form (PDF)  to the applicant.


Apply

Upon completion of the application, the applicant should mail the application to:

The Battle Creek Mobility Manager will make the eligibility determination.

Approval

If eligibility is granted, the Mobility Manager will send the applicant an approval letter with information about Battle Creek Transit’s complimentary paratransit service. If eligibility is denied, the Mobility Manager will send the applicant a denial letter with information on the appeals process.

If Battle Creek Transit has not made an eligibility determination within 21 days of receipt of the application, Battle Creek Transit will grant presumptive eligibility to the applicant.

Battle Creek Transit will keep a log and file of all ADA applications.
