ADA Certification Process

ADA Health Care Provider Release Form

ADA regulations require an administrative process for individuals to request to be evaluated for ADA eligibility.

PROCESS:

Upon receiving a request for an ADA application, Battle Creek Transit will provide an application and an authorization of physician or health care provider release of information form to the applicant.

Upon completion of the application, the applicant should mail the application to:

The Battle Creek Transit Manager will make the eligibility determination.

If eligibility is granted, the Transit Manager will send the applicant an approval letter with information about Battle Creek Transit’s complimentary paratransit service. If eligibility is denied, the Transit Manager will send the applicant a denial letter with information on the appeals process.

If Battle Creek Transit has not made an eligibility determination within twenty-one days of receipt of the application, Battle Creek transit will grant presumptive eligibility to the applicant until a determination is made.

Battle Creek Transit will keep a log and file of all ADA applications.
